Bring Your Insurance Card
Bring your insurance card with you and have it ready to present to the office clerk at every visit. In an effort to reduce billing errors, insurance cards must be presented at every visit. Our clinics accept most insurance including Scott & White.
Notify Front Desk of Contact Change
Advise front desk of any address, phone number or billing change. If your contact or insurance information has changed please print and fill out the contact change form and present to office clerk at your visit. Download change of information form here.
Bring Your Prescriptions to Your Visit
Bring your prescriptions with you. If you are taking any prescriptions, please bring to you visit and present to your practitioner or nurse at the time of your visit.
New Patient Information
New patients must fill out a new patient packet. Download, complete and bring with you to your first visit. Download a new patient packet here.
Be On Time
Please be prompt. Arrive 10 minutes before your appointment to insure all paperwork and insurance is in order.
